1. Select your CLUB ENTRY - this will filter the athletes to only your entry

2. You will need to enter an email for verification that is associated with your registration

3. Your athletes should be listed alphabetically by first name according to level

4. You can SUBMIT the form at any time during your sizing process and come back later to finish your team

5. We will send you a copy of your submission for your records

6. Sizing due by October 15th
